A RoleLogic plugin that gates Discord roles behind a member-fillable form. Build forms with single choice, multi choice, dropdown, scale, short text, long text, number, email, date, agreement, and info/image/video display blocks. Quiz mode auto-grades answers server-side and shows respondents a pass/fail result; surveys grant a role on any submission. Configure conditions per-question (equals, not equals, contains, regex, between for numeric ranges) or on quiz total score, then RoleLogic grants the Discord role on submit. Built-in anti-cheat: correct answers and point values are server-side only and never sent to the browser, question order and option order shuffle per page load, quizzes are one-shot with no edit-loop oracle, and the submit endpoint is CSRF-protected. Optional Discord-membership requirement, minimum Discord account age gate, scheduling (opens-at / closes-at), Discord webhook on each submission, admin response dashboard with CSV export and per-question columns, draft auto-save, and Discord OAuth identity binding. Perfect for server applications, rules-acknowledgement quizzes, age gates, role applications, partner/whitelist forms, member onboarding surveys, knowledge-check quizzes, and any Discord role that should be earned by answering correctly or simply by submitting a form.
